Scheme Content:
- Convenient screening services for diabetes mellitus (DM) and hypertension
- Received personalised and comprehensive primary healthcare services by preferred Family Doctor
- Integrated Care by Distrist Health Centre Professional Team
Eligibilty
- Hong Kong residents aged 45 or above*
- Not known medical history of diabetes mellitus or hypertension
- Have enrolled in the eHRSS and registered as a DHC/DHCE member
^During the screening stage, the government will provide a one-time subsidy of $196 for related expenses, and participants only need to pay a one-time co-payment of $120.
*(1) A person who holds a Hong Kong Identity Card issued under the Registration of Persons Ordinance (Chapter 177), but if the person is issued a Hong Kong Identity Card by virtue of having been granted permission to enter or stay, and such permission has expired or is no longer valid; or (2) holds a "Certificate of Exemption from Registration" within the meaning of the Immigration Ordinance (Cap. 115)

Family Doctor
Screen and arrange blood test(s) at designated medical laboratory
Explain investigation report and diagnosis
Treatment Phase:
Diabetes Mellitus / Hypertension / Pre- Diabetes Mellitus
Family Doctor continue manage Scheme Participant's chronic diseases
- Consultation
- Prescribe medication
- Arrange necessary laboratory tests and examinations as required
- Nurse Clinic services
- Allied health services
(Optometrists / Podiatrists / Dietitans / Physiotherapists)
- Health management groups
