Membership Eligibility
An individual who
-
Is a holder of the Hong Kong Identity Card issued under the Registration of Persons Ordinance (Cap. 177, the Laws of Hong Kong) or the certificate of exemption , except those who obtained their Hong Kong Identity Cards by virtue of a previous permission to land or remain in Hong Kong granted to him and such permission has expired or ceased to be valid; or a child who is a Hong Kong resident and under 11 years of age;

Agrees to enrol to eHRSS; and

Lives or works in Tuen Mun

*To enjoy these services, members need to complete a Fitness Assessment and a Fitness Workshop
#Members will be assessed by a registered nurse or member of the medical staff of the District Health Centre and will be given access to suitable Chronic Disease Management Programmes and/or Community Rehabilitation Services, as required. They can come to the Centre for personalised health risk screening, or be referred by：
- The Hospital Authority；
- Network M & H Practitioner
Registration Procedure
Please bring the following documents to the Centre to register as a member:
Identity documents:
- Hong Kong Identity Card
- Residential or working address proof in Tuen Mun
Medical waivers for verification, if applicable:
- Comprehensive Social Security Assistance
- Old Age Living Allowance (Age 75 or above)
- Residential Care Service Voucher (Co-payment Level 0）
- Medical fee waiver issued by the Hospital Authority.
