Participate in the "Chronic Disease Co-treatment" pilot program and receive convenient diabetes and hypertension screening services.
Tailor-made health management plans with the District Health Center to control risk factors for chronic diseases and prevent them early.
Prevent chronic diseases and reduce related complications!

Scheme Content:

  • Convenient screening services for diabetes mellitus (DM) and hypertension
  • Received personalised and comprehensive primary healthcare services by preferred Family Doctor 
  • Integrated Care by Distrist Health Centre Professional Team

Eligibilty

  • Hong Kong residents aged 45 or above*
  • Not known medical history of diabetes mellitus or hypertension
  • Have enrolled in the eHRSS and registered as a DHC/DHCE member

^During the screening stage, the government will provide a one-time subsidy of $196 for related expenses, and participants only need to pay a one-time co-payment of $120.

*(1) A person who holds a Hong Kong Identity Card issued under the Registration of Persons Ordinance (Chapter 177), but if the person is issued a Hong Kong Identity Card by virtue of having been granted permission to enter or stay, and such permission has expired or is no longer valid; or (2) holds a "Certificate of Exemption from Registration" within the meaning of the Immigration Ordinance (Cap. 115)
