Personal Information Collection Statement

We, the Tuen Mun District Health Centre (“TMDHC”) commits and takes reasonably practicable steps to protect the privacy, confidentiality and security of the personal information collected from a client in the DHC Scheme, in accordance with the requirements of the Personal Data (Privacy) Ordinance (PD(P)O) (“The Ordinance”).

Purpose of Personal Data Collection

TMDHC collects an appropriate amount of, but not excessive, personal data by fair and lawful means.

You are invited to provide personal data through this TMDHC website/ mobile app, on a voluntary basis. The data so collected are for the services and events of TMDHC.

When you provide Personal Data to us, please make sure that the data are accurate and complete, and notify TMDHC if there are any changes. Failure to provide accurate/complete information may affect TMDHC’s ability to process your application or provide healthcare services.

Use of Personal Data

Personal data provided will be used by TMDHC for one or more of the following purposes:

1. Processing membership applications
2. Verification
TMDHC staff will collect personal data when people register online or in person. The data will used to identify the persons concerned when they use the mobile app or website, and to contact them if necessary
3. Feedback
TMDHC collects feedback for research purposes and to help optimize our services and events.
4. For Direct Marketing, according to your preference (please refer to the “Use of Personal Data in Direct Marketing”)
5. Such purposes are permitted by or under the laws of Hong Kong e.g. court proceedings and criminal investigations

Data Accuracy

TMDHC will take reasonable and practicable steps to ensure the accuracy of personal data.

Data Storage

Files and records containing personal data will not be used after fulfilling the purpose for which they were collected.

Data Security

TMDHC will take appropriate steps to protect the personal data we hold against loss, unauthorized access, use, modification or disclosure.

Data Sharing with Third Parties

Personal data will only be accessible to TMDHC staff and will be disclosed to third parties only when necessary:

1. Personnel or organizations to which you have agreed to disclose data.
2. Related parties that are authorized by law, or with which TMDHC is required to share data.
3. TMDHC will not sell, rent or submit personal data to other organizations.

Access and Correction of Personal Data

You have the right to request access to, or correction of, personal data, except in cases specified by the Ordinance. A fee will be charged for any data access request. Please email the completed request form to the Administration Manager, TMDHC ( tmdhc@elchk.org.hk) or post to 1/F, Rosedale Gardens Shopping Mall, 133 Castle Peak Road, San Hui, Tuen Mun.

Use of Personal Data for Direct Marketing Purposes

TMDHC will use your personal data (including name, mobile number, fax, email and mailing address) as appropriate to update you on the work of TMDHC, including member and volunteer recruitment, fundraising, courses, events, and e-news via direct messaging (eg. WhatsApp, SMS, Wechat), mail, email and telephone.

TMDHC will only use your data for such purposes with your consent. You have the right to request TMDHC to stop using your personal data for direct marketing purposes (see below).

Opting out of Direct Marketing

TMDHC respects your right to opt out of direct marketing. You can request TMDHC to stop using your personal data at any time by notifying us in writing. TMDHC will then remove you from our mailing list.
